INSIGHT LAB, Inc. ("we", "our", or "us") main business activities are system planning/design/development, consulting, and website development. Personal information that we obtain from our customers and employees through our business activities ("Personal Information") is used for the following purposes. The Company recognizes that the protection of Personal Information is an important social responsibility of the Company. Therefore, we are committed to fulfilling our social responsibility regarding the protection of personal information by providing "peace of mind" to our customers and employees by handling personal information acquired through our business activities under the following policy.

• We collect personal information through legal and fair means.
• We use personal information only to the extent necessary to achieve the purpose of use.

• We conduct safety inspections. Then we promptly correct any violations, incidents, or accidents that are discovered to strengthen its precautions.
• We thoroughly give the employees and business partners training on safety.

5.Continuous Improvement

• Our personal information protection management system is monitored and audited for compliance with internal rules to achieve our personal information protection policy and is reviewed by management to detect violations, incidents, accidents, and weak points. It is reflected in management measures and internal regulations to improve the system continuously.

【Procedure for Requesting Disclosure, And So Forth.】

The procedures for responding to a request for disclosure, and so forth, from the individual or his/her deputy concerning the personal information held by us are as follows.

1.Contact for Disclosure Request of Personal Information

To request for disclosure of personal information, please send us the request form with the necessary documents.
Please use a method that allows us to confirm a delivery record, such as delivery recorded mail or registered mail, when you send us your request form.
We would appreciate it if you wrote "Personal Information Request Form enclosed" in red ink on the envelope.

 

2.Necessary Documents to Submitted for a Request for Disclosure.

To make a request for disclosure and so forth, please send us a Personal Information Disclosure Request Form that is filled in all the prescribed items, with one of the following documents that can verify your identity.

• Driver's License

• Copy of Certificate of Residence

• Health Insurance Card

※Please black out the Permanent Domicile

 

3.Request for Disclosure, and so forth by Representative

If you request to authorize a representative to make a request for disclosure and so forth, please contact the Personal Information Manager.

 

4.Handling Charge for A Request for Disclosure, And So Forth, and Notification of Purpose of Use

A fee of 1,000 yen (tax included) will be charged for each request for disclosure of personal information and notification of purpose of use.

Please enclose a 1,000 yen postal money order with your submission documents.

Customers are responsible for the cost of purchasing a postal money order and postage to us. If the fee is insufficient, or if the fee is not enclosed, it shall be deemed that no request.

 

5.How to Response to Requests for Disclosure, And So Forth.

We will send the answer in writing to the address of the requestor stated on the request form.

6.Revisions

In order to protect your personal information and comply with changes in laws and regulations, the contents of this request procedure may be partially revised. Please check this procedure each time you make a request for disclosure and so forth.
